Question: Can this be used to connect a Focusrite 4i4 to an Behringer HA400 headphone amp?

Answer: The HA-400 has a single stereo TRS input. If you'd like to run the headphones in stereo, use a TRS-TRS 1/4" cable

I have a BX5 D2 which requires two mono 1/4" jacks to 3.5mm to connect directly to my motherboard. It still holds very well.
